The Higher Form/Seeking Nomination Questionnaire will need to be completed if a member is
seeking a nomination for a DAV Department of Michigan Officer position.
- Please ensure to complete all questions on the form
- A photo (4x6, wallet size or, if needed, a passport photo) should be uploaded with the form
- Upon receipt of your Higher Form, the Department will email you confirmation of receipt of your nomination request
- Completed form is due May 6, 2026 (per the Constitution, Article IV.)
- Nomination interviews will occur on Wednesday, June 3, 2026 at the convention. You will be notified of your interview time prior to the convention.
